We are currently accepting new patients who live within our practice area (See Practice Boundary from list on right).

Please ensure that complete the above forms and bring them to the surgery along wth your proof of identification as below.
Due to new recommendations from NHS Counter Fraud Service we now ask patients to provide proof of identification:
You will need to bring along two documents:
One needs to be a Birth Certificate/Marriage Certificate/Medical Card/Driving Licence (photographic)/Passport.
The other can be Local Authority Rent Card/Paid Utility Bill/Bank/Building Society statements/Payslip/Letter from Benefits Agency/Benefit book/Signing on card/Papers from the Home Office/P45 or any other acceptable identification.
Please do not send this information by post as we cannot be responsible for any loss of documents etc.
If you move out of our area, or wish to change doctors for any reason, your medical card explains how to do this.
Change of Address
Remember to keep us informed if you move house or change your name, including family members registered at the practice. This will ensure that your records are always up to date and confirm that you are still living in our practice area. A contact number is also useful.
